We are proud to offer a monthly wellness and maintenance
program for all of our patients and members of the community.
Membership is only $25 per month.

Company history
Founded as a corporation in October 2007, Five Star Physical Therapy, Inc.  came to fruition in February 2008 after construction of a brand new office space and set-up of state of the art medical and exercise equipment.
